Job summary
The Urgent Care Team is a 24/7 service; providing a firstpoint of contact and triage service, including telephone and face to faceclinical assessment of patients and/or their carers/relatives, resulting inappropriate decision making and plan of care. The Team work closely with otherdisciplines including GPs, District Nurses, Mental Health Services, AdultSocial Care, End of Life Care Services, Paramedics and other externalproviders.
The Triage Clinicians, as part of the Urgent Care Team, area dynamic group of Nurses and Paramedics with a diverse range of experience andbackgrounds, which responds to the unplanned needs of the patients in NorthEast Lincolnshire.

Salary Range 5£28,184 -£32,676 (Pay award pending) pro rata per annum
We are a friendly, supportive team of clinicians providing aClinical Assessment Service to the residents and visitors to North EastLincolnshire. You will be part of the development of the service which hasalready grown both in capacity and scope over the last 2 years. We play apivotal role in the support of community healthcare and hospital avoidance. Weare unique as a service being able to provide a direct access point for bothprofessionals and members of the public with all clinicians having an understandingof the area/services available and social/economic considerations ensuring aholistic approach is maintained. The service is proactive and works closelywith other organisations to pilot new, innovative strategies.
By utilising agreed protocols and policies in conjunctionwith listening, questioning and probing skills you will support in planningindividual patient care. You will complete clinical assessment of minorailments through to life threatening emergencies and arrange appropriate andsafe Ambulance and GP attendance, if required.
