Primary Responsibilities:
Prioritizes and responds to calls received, in a timely manner. Incorporates nursing skills and appropriate questions to accurately evaluate problems/needs and instruct the patient/care giver in accordance with the plan of care. Provides emotional support appropriately during the telephone conversations to patients/caregivers. Determines appropriate discipline to respond to call and/or visit patient/family. Conducts follow up calls as problems/needs arise or as requested by the team members.
1. Communicates/documents with staff, physicians, community resources, vendors, providing accurate information regarding patient’s needs/problems.
2. Documents all telephone communications with patients/caregivers via computer in a timely manner per CHNE protocol.
3. Receives daily team reports and updates from primary nurses.
4. Maintains and audits hard copy of patient information as needed.
5. Fulfills mandatory skills training, as required.
6. All other duties as assigned.
Required Licenses/Certifications:
7. Must currently be registered in the State of Florida as a Registered Nurse.
8. Current State of Florida drivers license with driving record acceptable to Community Hospice insurance carrier.
Qualifications:
Agency for Healthcare Administration (AHCA) Level II Background Screen
Education/Experience:
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
9. Possession of a degree from a diploma nursing program, 2 year Associate Degree, or Bachelor’s Degree in Nursing
10. Minimum of two (2) years of experience in medical-surgical, oncology or home health setting.
