What the Job Involves As a Specialist Resettlement Housing Coordinator, your primary role is to provide specialist casework for single homeless applicants, specifically those leaving institutions such as hospitals and prisons, and those with complex support needs. You will ensure compliance with the Homelessness Reduction Act through proactive, person-centred, and outcome-focused case management. Your role involves undertaking comprehensive assessments to create focused Personal Housing Plans (PHP) and navigating prevention and relief duties. You will assess and complete high-quality risk management plans, report safeguarding concerns, and maintain accurate, GDPR-compliant client records. You will collaborate with various internal and external partners, attend multi-agency meetings, and contribute to service development within the Housing Advisory Service. Additionally, you will participate in creating and deploying innovative approaches to improve housing sustainment and offer robust support to the Resettlement Team.
