The ideal candidate will be forward thinking, be able to work as part of the team and on their own initiative and be confident in their decision making ability. You will be able to demonstrate knowledge of and commitment to the personalisation agenda including Self-Directed Support and co-production. Under the supervision of the Team Manager you will undertake a wide range of tasks and responsibilities including Care Management responsibilities for an assigned caseload, participating in the service's duty system and carrying out assessments to ensure that outcomes are being identified and met. In order to support work/life balance, applications from those wishing to work part time will be considered.
