You are adept at working with young adults aged 18-24 who were previously looked after by local authorities. What the Job Involves This role is focused on ensuring the council meets its statutory obligations to individuals leaving care. You will manage a caseload of up to 27 young people, working directly with them to develop and implement tailored care plans. You will collaborate with a multi-agency network to provide comprehensive support, participate in group work sessions, and respond to emergencies. Your responsibilities also include financial planning and coordination with external agencies to secure accommodation, education, and legal services.
