CDDFT Community Respiratory Nursing Service is pleased to advertise a rare, 1 year, exciting secondment opportunity to join the Community Respiratory Specialist Nursing service as a staff nurse working across North Durham and develop skills in respiratory nursing including home oxygen review.
The successful postholder will deliver respiratory care in out-patient clinics as well as in their own homes, supporting the specialist respiratory nursing service.
The successful candidate will utilise their outstanding organisation and communication skills to achieve best patient outcomes whilst being well supported by expert respiratory specialists to develop a range of competencies in delivering excellent respiratory care and support for this group of complex patients.
The successful candidate will receive mentorship and opportunities to develop their learning and skill in the specialist field.
This post offers weekday working and flexible hours can be negotiated.
Main duties of the job
1. To support the respiratory specialist nursing team to manage complex respiratory patients in the community.
2. Support community teams with complex respiratory patients.
3. Take responsibility for the assessment of the health needs of patients under community respiratory care and manage a caseload of respiratory patients.
4. Offer chronic disease management of respiratory patients including COPD exacerbation pathway patients and virtual ward.
5. Signpost patients and colleagues to appropriate services and escalate as needed.
6. Provide specialist advice to patients/carers.
7. Maintain records.
8. Accept responsibility for decisions made and actions taken.
9. Skilled at assessing and managing risk.
About us
We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities
MAIN DUTIES AND RESPONSIBILITIES
1. Co-manage a caseload of complex respiratory patients, under direction of the community respiratory nursing team, across the community respiratory specialist service, within clinic and patients home environments, Monday-Friday 9am-5pm.
2. Undertake holistic patient respiratory assessment and review including social and psychological well-being and progress treatment interventions and adapt interventions, within set protocols, to meet the changing needs of the patient.
3. Develop, implement and evaluate patient treatment plans.
4. Communicate patient treatment plan changes to respiratory specialist nursing team.
5. Understand basic respiratory anatomy and pathophysiology and the function and management of all respiratory conditions including mental well-being.
6. Perform capillary blood gas testing and oxygen reviews.
7. Accurately record clinical data and understand its significance including heart rate and rhythm, respiratory rate, pulse oximetry, peak flow rate, and blood pressure.
8. Complete effective record keeping which demonstrates the decision-making process, care planned and given, as well as patient outcomes in accordance with trust policy and legal requirements.
9. Understand the benefits of non-pharmacological interventions for respiratory illness/conditions including smoking cessation, weight management and nutrition, physical activity and brief cognitive behavioral therapy (CBT).
Person Specification
Experience
* Up to date knowledge of professional practice.
* Post registration experience of working with respiratory patients.
* Experience of working in a community setting.
Statutory Registration
* Nursing Midwifery Council part 1 and 2 Registration.
Qualifications
* Registered Nurse.
* Professional knowledge acquired through degree or equivalent level of demonstrable professional knowledge/experience.
* Cognitive Behavioural Therapy qualification.
* Accredited respiratory course.
Special Skills & Knowledge
* Excellent communication and interpersonal skills, organisational and problem-solving skills.
* Ability to undertake holistic respiratory assessment and review or willingness to learn.
* Ability to work independently and act alone in an emergency situation.
