Your role will involve conducting thorough assessments, delivering effective treatments, and offering brief, evidence-based follow-up services to patients referred from community mental health teams, the psych liaison service, or inpatient wards.

The post holder will support the CRHTT Associate Nurse Consultant, Co-occurring Mental Health Alcohol & Drugs, to support the cohort of patients who are experiencing a mental health crisis which may be complicated by alcohol or substance dependence.
The post holder will also support the team in outreach work for those in mental health crisis which may be complicated by homelessness or unstable housing, ensuring those identified as requiring crisis mental health support are afforded the same opportunities as those where home treatment if the first line intervention available.
Whilst working under the supervision of the Associate Nurse Consultant, this role will hold a degree of autonomy and experience in working with this patient cohort will be key in ensuring success.
Conduct assessments and care planning thatdemonstratesa high levelofexpertisein substance and alcohol related complexities for people experiencing mental health crisis.
Develop and expand on relationships with locally commissioned substance misuse services and Local Authorities and VCSE partners to ensure patients receivea holistic approachwhen managing their mental health needs.
The post holder will review service users to the service, who are in a mental health crisis and require intensive home treatment and support without which they wouldlikely requirea hospital admission

The "must haves" for this role:
• Extensive experience of working with patients who needs arepredominantly substancemisuse
• Experienceofworkingwithpatient who are homeless
• Evidence of a soundunderstanding of theacute care pathway.
• Experienceofassessment and case formulation, evidence of skills utilised to manage risk.
