The Role
An opportunity has arisen for a Social Worker - Social Care Navigator within the Intermediate Tier Services. This service covers Admission Avoidance; Intermediate Care and Discharge to Assess bed-based units.
The post of Social Care Navigator receives a 7% enhancement as workers are required to work weekends and bank holidays on a rotational basis. This is to support the delivery of the service 365 days of the year
This post offers an opportunity to contribute to the ongoing development of innovative ways of working across health and social care in order to improve the experience of service users and carers and to promote well-being and independence.
The ability to develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in inter-disciplinary team working.

About You
We are looking to recruit an enthusiastic and motivated individual who has a passion for working with adults, understanding their needs, aspirations and supporting them to enhance their quality of life and promote independence.
As a Social Care Navigator in IMC you will work in one of the designated services and undertake assessments/reviews of need under the Care Act 2014, assist with support planning as necessary and also link people in to universal and voluntary services. You will also assist colleagues within other areas of the services as necessary and support with complex assessments, mental capacity and best interest assessments.
