The pharmacy department within Kent Community Health NHS Foundation Trust are looking for Pharmacist colleagues who are enthusiastic and keen to develop their skills further in the Community Hospitals setting.
For those Band 6 Pharmacist colleagues who are looking to progress to a Band 7 role but require a little more support or do not have Hospital backgrounds, at KCHFT we would like to support you with this aspiration by offering the right applicant a Band 6 to 7 progressive post.
We are committed to developing our staff and offer support to complete the post graduate clinical diploma and independent prescribing courses.
We are looking for resilient Pharmacists who are willing to work across our community hospitals in Kent and thrive on the variety of work a community Trust brings.

We can offer full time or part time hours; these can be negotiated.
Main duties of the job
As part of the Pharmacy team, you will be responsible for providing safe and effective medicines optimisation services to a number of community hospital sites in person and remotely.
You will develop an expertise in the optimisation of medicines in frailty, working with multidisciplinary teams to ensure that we achieve the best medicines related outcomes for the patients we serve.

Person Specification
Qualifications
* MSc in Pharmacy (entry level for pharmacy). Registration with General Pharmaceutical Council.
* Postgraduate Clinical Diploma Independent Prescribing qualification
Experience
* Post registration community or hospital experience
* Understanding of practical medicines management issues in community based services.
* Experience and knowledge of hospital practice including dispensing and clinical experience.
* Clinical experience of End of Life Care
* Experience of whole system working and collaboration
Knowledge
* Knowledge of risk management and assurance issues relating to all aspects of medicines use.
* Familiar with the concepts of evidence-based medicine.
* Understanding of the concepts of Clinical Governance.
* Knowledge and experience of provision of pharmacy services to community services
* Clinical Knowledge of medicines in frailty.
Skills & abilities
* Able to work independently and on own initiative.
* Positive and forward- looking attitude.
* Ability to work at interfaces between primary, secondary, community and social care including intermediate care.
* Excellent written and verbal communication skills to communicate highly complex information where there may be barriers to understanding, and be able to motivate, challenge and negotiate within the team and with others.
* Excellent inter-personal skills including the ability to challenge others and be challenged.
* Ability to extract and present data from Datix
