Job overview
An exciting opportunity has arisen for a dynamic, motivated Nurse or Pharmacy Technician to join Neath Port Talbot Medicines Management Team for Domiciliary Care.
The successful applicant will be part of the integrated, multidisciplinary Community Resource Team who provide support and education in medicines management, specifically in domiciliary and intermediate care settings. This role aims to improve patient outcomes and minimize harm, including avoiding unscheduled care, admissions, and delays to transfers of care. It is ideal for individuals interested in integrated working and are committed to developing and expanding their skills to achieve the highest quality care and outcomes for patients in their own homes.
Main duties of the job
The role includes home visiting; providing medicines management interventions to patients within their own home, involving lone working without direct supervision. You will liaise with a wide range of Health & Social Care professionals to identify solutions for patients in a multidisciplinary network with a focus on promoting independence, prevention and early intervention. Delivering medicines management training to relevant staff groups will also be included, and participation in audit processes.
The post holder must be able to travel around the locality.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Essential Qualifications & Knowledge
* Registered with relevant professional body such as NMC or GPhC
* Specialist knowledge acquired through health-related qualification such as BTEC science (pharmaceutical) Registered Pharmacy Technician, equivalent level qualification plus specialist training and experience to degree or equivalent level
* Knowledge and evidence of Professional requirement for ongoing Continuing Professional Development to maintain knowledge and expertise
* Further post-graduate management or clinical qualification
* Experience of facilitating change to improve clinical practice
* Experience and good working knowledge of clinical audit
Essential Aptitude and Abilities
* Adhere to and can demonstrate SBU Values & Behaviours
* Ability to define, collate, analyse and interpret data in a timely manner to solve complex problems and plan effectively, and to motivate people and facilitate change
* Ability to provide medicines related information to a range of health professionals, managers, social workers, professional carers, and other health care staff, overcoming potential barriers to understanding
* Ability to provide medicines related information to patients and relatives, requiring well-developed empathic and reassurance skills
* Effective interpersonal, communication (both written and oral), presentation and influencing skills
* Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh
