Care co-ordinators have a varied and interesting role. They are here to support new diagnoses of cancer, the bereaved patient, new discharges from hospital, and can hold surgeries themselves to meet patients and signpost them to other agencies.
This really is an exciting and very rewarding role.
Main duties of the job
* Welfare checks on frail and vulnerable patients
* Facilitate group consultations
* Meet patients in a surgery session to signpost to various support services
* Make contact and offer support to newly diagnosed cancer patients and newly bereaved.

East Trees Health Centre is a beautiful modern building housing support services such as Midwife, Speech and language, Podiatry, MSK, Community Nursing, Health Visitors.
Our team comprises 4 partners, 7 salaried GPs, a nursing team, paramedic, nurse practitioner, admin and care navigation team.
We are a GP/Nurse training practice and have 15,600 patients to look after. We all work closely together in a friendly non-hierarchical way and opinions are listened to. We value the health and well-being of our team and work closely together to provide the best care we can to our patients.

Person Specification
Qualifications
* GCSE grade A-C (or equivalent)
* Excellent telephone manner
* Experience of working in Primary care or other health care setting
* NVQ level 3
