Pharmacist (Primary Care) - Mountain View Health Centre
Join our friendly team as a Part-Time Pharmacist that can prescribe at Mountain View Health Centre.
The hours are Monday to Friday, 30 hours per week, 6 hours per day.
The Prescribing Pharmacist will play a pivotal role in enhancing the level of care provided to patients through advanced practice interventions. This role involves autonomously managing complete episodes of clinical care across all age groups and demographics, encompassing a wide range of clinical conditions, diagnosing, treating, and referring patients as appropriate. The Pharmacist will operate within a multi-disciplinary team and is instrumental in mentoring and supporting both nursing staff and trainee practitioners.
Key responsibilities include the application of advanced clinical skills to assess, diagnose, treat, prescribe, and advise patients across a variety of settings including face-to-face, telephone, and e-consultations, ensuring the use of evidence-based practice. The Pharmacist will also lead in the development of new services and pathways to improve the quality of care and patient outcomes. This role requires dynamic clinical leadership and activities to promote a culture of learning and continuous improvement within the practice.
Main duties of the job
Clinical Responsibilities:
1. Patient Care: Deliver advanced and complex clinical care.
2. Autonomous Practice: Work independently within the scope of practice to manage complete and undifferentiated clinical episodes.
3. Case Management: Manage patient cases from initial assessment to conclusion.
Leadership and Guidance:
1. Mentorship: Provide mentorship and supervision to nursing staff.
2. Service Development: Lead the development and implementation of new clinical services.
3. Professional Development: Engage in continuous professional development.
Research and Quality Improvement:
1. Research Participation: Participate in clinical audits and research activities.
2. Quality Assurance: Implement quality improvement initiatives.
Educational Activities:
1. Training Provision: Deliver training sessions to healthcare staff.
2. CPD Activities: Facilitate and participate in Continuous Professional Development (CPD) activities.
Administrative and Organisational Duties:
1. Record Keeping: Maintain comprehensive records using the practice software.
2. Meetings: Attend and contribute to team meetings.

Person Specification
Experience
* Experience of working within a pharmacy team and preferably at a GP surgery.
* Experience working with vulnerable patients.
* Previous experience in a GP surgery/Primary care in the UK, with a focus on managing long-term conditions and minor illnesses. Ability to work both autonomously and collaboratively within a multidisciplinary team.
Qualifications
* GPhC | Professional Standards Authority Independent Prescriber qualification. Health and Care Professions Council (HCPC) or other regulatory body as appropriate.
