Job summary We currently have an exciting opportunity for highly motivated and enthusiastic ACP/Trainee ACP to work within our Acute Home Visiting service (Team Up) in Swadlincote. A Band 7 trainee position may be offered to an aspiring Advanced Clinical Practitioner who is committed to undertaking an apprenticeship training programme. A Band 8a position will be offered to candidates who have already completed an MSc in Advanced Practice (or equivalent) and who can demonstrate contemporary experience working at an advanced level within a relevant clinical environment. Main duties of the job The successful candidate will support patients referred to the Acute Home Vising service in Swadlincote primarily ensuring patients remain safe in their own home. As an ACP you will: Provide clinical leadership, including direct clinical care, and advice on complex issues Establish a cohesive team approach for the ACP workforce development Lead on training needs discussions, based on audit findings, supervision outputs and individual staff knowledge gaps Assist in feedback for staff appraisals. You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to patients in their own homes. You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of community patients. You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership. You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.
