Job summary An exciting opportunity has arisen to be part of an experienced team of complex discharge coordinators across Greater Manchester's inpatient Neurological Rehabilitation services. These services include hyper-acute neuro-rehabilitation beds at Salford Royal NHS Foundation Trust, as well post-acute beds at Trafford INRU. You will be based at Salford and directly report to the Band 7 Complex Discharge Team Leader. Successful applicants will need a full driving licence and be expected to travel to the other units to provide cross cover as required. Main duties of the job An exciting opportunity has arisen to be part of an experienced team of complex discharge coordinators across Greater Manchester's inpatient Neurological Rehabilitation services. These services include hyper-acute neuro-rehabilitation beds at Salford Royal NHS Foundation Trust, as well post-acute beds at Trafford Hospital, the Devonshire Centre (Stockport) and the Floyd Unit in Rochdale. The Neuro Rehabilitation Complex Discharge Service is a specialist, multi-disciplinary team of complex discharge coordinators to help ensure that delays to discharge from these inpatient settings are minimised. You will work as part of dedicated multidisciplinary integrated discharge service to start planning discharge from the earliest opportunity with the needs of the patient at the centre of decision making. You will actively lead and manage the coordination of patients who require safe and timely discharge from the Greater Manchester Neuro-Rehabilitation Inpatients services and where appropriate act as a Trusted assessor. You will have knowledge and understanding of the different processes and pathways to support a safe and timely discharge including the Discharge to Assess pathways as well as more complex discharge planning. You will provide focused support for patients and carers to ensure they are in the most appropriate place for their care and facilitated discharge.
