Main duties of the job
To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such, health promotion, chronic disease management, health prevention, well women/well man clinics, as well as routine practice nurse duties. Supporting the management team in the reviewing of clinical policy and procedure.
14. Vaccinations - Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions (essential)
15. Long Term Conditions Management Asthma, Diabetes, COPD, CVD, Learning Disabilities, Anticoagulation monitoring (essential)
16. Travel Health - Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, food hygiene, sun protection.
17. Health Promotion & Screening Cervical Smears, Sexual Health & Family Planning, Contraception pill checks/coil checks.
18. Well Person Clinics To provide a holistic assessment and lifestyle advice on diet, smoking, alcohol intake & exercise.
19. Perform venepuncture according to local guidelines.
20. Assist in the provision of minor surgery and coil clinics
21. Ensure infection control guidelines are maintained.

The following are the core responsibilities of the practice nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

35. Deliver and maintain primary nurse led services including long term conditions in line with disease registers.
36. Ability to monitor and manage maintenance of stock and equipment to include refrigeration, sterilizer and emergency equipment.
37. Data protection and confidentially of information gained at work must be preserved at all times. Adhere to policies relating to GDPR.
38. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
39. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
40. Support the clinical team with all safeguarding matters, in accordance with local and national policies
41. Monitor and ensure the safe storage, rotation and disposal of medicines/vaccines
42. Support junior members of the nursing team, providing guidance when necessary
43. Participate in local initiatives to enhance service delivery and patient care
44. Support and participate in shared learning within the practice
45. Continually review clinical practices, responding to national policies and initiatives where appropriate
46. Participate in the review of significant and near-miss events applying a structured approach root cause analysis (RCA)
47. Maintain accurate and contemporaneous records using the Practice clinical system (SystmOne)
48. The ideal applicant will be flexible, and able to provide annual leave and sickness cover to the nursing team team
