This involves coordinating with various healthcare professionals, departments, or external sources. Adding Clinical Diagnosis to Patient Records: As part of maintaining accurate and up-to-date patient information, you will add relevant clinical diagnoses to patient records based on incoming letters and other documentation. This requires attention to detail and knowledge of medical terminology. Reading and Directing Required Actions on Incoming Clinical Letters: You will review clinical letters to identify any necessary actions, such as follow-ups, referrals, medication changes or updates to patient care plans. You will direct these actions to the appropriate medical staff or departments. Managing Patient Record Transfers: This includes coordinating the transfer of patient records, moving patient data between departments or healthcare providers. l. Summarising and Coding Patient Records: The role involves summarising patient information and accurately coding medical records for data entry into systems. This requires knowledge of coding systems and an understanding of patient care documentation.
