Performs skilled nursing activities such as:
* Actively receive and process incoming calls for all lines of business.

Communicates and coordinates as follows:
* Provide change of status updates to supervisor (such as ER visit disposition or death) per protocol.
* Call physician or APRN as necessary for change in plan of care.
* Collaborate with supervisor or IDG team to determine appropriate discipline to respond to call and/or visit need.
* Arranges for medications and supplies based on urgency, per policy.
* Communicates with staff, physicians, community resources, and vendors providing accurate information regarding patient’s needs/problems.
* Documents all phone call encounters with patients/caregivers in the EMR in a timely manner per CHPC protocol.
* Leaves accurate and detailed report, per protocol, to IDG team members, after all calls including death calls, per protocol.
Actively seeks ways to develop professionally:
* Develops familiarity and proficiency with difficult discussions, explaining disease progression, end of life needs and expectations, and use of current CHPC materials for family/caregiver education.
* Develops basic knowledge of Hospice Medicare/Medicaid and other reimbursement mechanisms.
* Develop areas of expertise in hospice nursing skills (including assessment and symptom management)
* Pursues professional growth through academic and continuing educational programs.

Qualifications:
Required Licenses/Certifications:
* Must currently be registered in the State of Florida as a Registered Nurse.
* Current State of Florida drivers license with driving record acceptable to Community Hospice insurance carrier.
* Reliable transportation
Education/Experience:
* Possession of a degree from a diploma nursing program, 2-year associate degree, or bachelor’s degree in nursing
* Minimum of two (2) years’ experience in acute care setting, oncology, home health setting, and hospice preferred.
* Principles and practices of nursing.
Knowledge of:
* Principles and practices of nursing.
