Job Opportunity - Social Worker – Chesterfield Long trm Adult Care Team
About Us
Join our dedicated team at Derbyshire County Council, with an exciting opportunity to join the Chesterfield Long term Adult Care team. We have various posts including part-time, full time, permanent and on a temporary basis.
We work with adults who have care and support needs, including those approaching adulthood, both in the community and in care placements. Our approach is rooted in person-centred assessments, with a focus on Strength Based Models of practice and principles outlined in the Care Act 2014.
Key Responsibilities
- Conduct person-centred assessments, working closely with individuals and their carers to gather information and positively influence wellbeing.
- Undertake personalised safeguarding enquiries, demonstrating strong communication and organizational skills.
- Collaborate with other organizations and professionals, fostering effective multi-disciplinary practice.
- Support individuals in maximizing opportunities for choice and control over their lives.
