Cardiac Rehabilitation Clinical Nurse Specialist

An exciting job opportunity has arisen within the ABUHB Cardiac Rehabilitation Service for a Band 6 Clinical Nurse Specialist to join our dynamic innovative team at Nevill Hall Hospital. We are looking for someone who is motivated, enthusiastic and passionate about delivering high quality care and supporting cardiac patients following acute cardiac events and interventions, cardiac surgery, or diagnosis of heart failure. We pride ourselves on innovation and continued service improvement to ensure that cardiac patients are always in receipt of the highest standards of cardiac care.

Main duties of the job
1. To assist in providing an integrated cardiac rehabilitation (CR) service primarily for residents within ABUHB, working closely with primary and secondary care.
2. To provide care for patients in a holistic manner incorporating physical and psychological care to enable them to live with a cardiac condition.
3. To work as an independent and autonomous practitioner within a multidisciplinary team, ensuring that patients with Coronary Heart Disease (CHD), those recovering from cardiac surgery, ICD implantation, or diagnosed with Heart Failure, receive the highest standards of care and comprehensive support from diagnosis through every stage of their condition.
4. Demonstrate excellent clinical assessment skills with a view to optimising patient's medication.
5. To provide professional support, clinical expertise and appropriate education and training to all those professionals involved in providing care to patients.
6. To assist in leading and developing CHD and Heart Failure care to meet the requirements of the NSF quality requirements and NICE/ESC guidelines ensuring a continuous process of evaluation.
7. Undertake an active role in promoting and carrying out research in conjunction with other health professionals - seeking to have work published.

Qualifications and/or Knowledge
1. Nursing Degree
2. Evidence of post registration development
3. Evidence of post reg cardiac course
4. Health promotion training
5. Post registration teaching/research module
6. Supplementary or Independent Prescribing certificate
7. Masters level qualification or working towards this
Experience
1. Post registration at band 5 grade or above in cardiac nursing environment
2. Experience of managing own work load
3. Experience in teaching and providing professional support to nurses and other colleagues
4. Experience of practice in a primary care setting
5. Health promotion/education or rehabilitation experience
