Our Team: Working within the Public Health Directorate, the Health Improvement Team provide a variety of services to the residents of Halton, encouraging and facilitating them to make healthier lifestyle choices. We work with the people of Halton to deliver these services; as such, this role does involve evening working. Your Contributions: You will support individuals in the community to make healthier lifestyle choices to improve their health and well-being; undertaking NHS Health Checks and Personal Health Plans as well as connecting individuals with mainstream services in Halton such as exercise, healthy eating, building confidence and self-esteem, social activities, learning new skills, drugs, smoking, and alcohol.
