Job summary To provide a high level of clinical expertise, to support and facilitate individualised patient centred care to patients with a life limiting illness and to provide care and support for relatives and carers in collaboration with the multi-disciplinary team. To facilitate and support the implementation of evidence-based practice and contribute to the on-going development of the Service. To participate in a seven day per week operational service. Main duties of the job Demonstrates leadership through clinical expertise and specialist knowledge, delivering high standards of personalised care. Identifying needs through a holistic assessment and co-creating a care and support plan with people living with cancer and other life limiting illnesses. To independently manage own caseload of patients and associated responsibilities. To independently manage the role of duty nurse on rotation with colleagues. To assess, and triage new patient referrals when required, in the absence of triage specialist nurse. To assess, plan, implement and evaluate all aspects of the patients care. To provide symptom control, social, emotional, and spiritual care for the patient, family, and carers. To monitor patients on an ongoing basis, to evaluate outcomes and adjust care plans to ensure optimum co-ordination and delivery of care. To deliver high standards of care directly and assist in the management of junior staff. To develop and maintain effective relationships with all members of the MDTto ensure collaborative working for the benefit of patient care. To support the development and implementation of care pathways and patient information. To maintain accurate and up to date patient records (including electronic records). To provide a high standard of personalised and on-going care for bereaved families. To ensure that the dignity, safety, and confidentiality of all patients is respected and that all patients receive the highest standards of care.
