Responsibilities Complete an initial 12week training and preceptorship, to become an autonomous assessor Conduct thorough assessments of patients to determine their healthcare needs and eligibility for support. Utilise your clinical knowledge of anatomy, medications, conditions and ADLs to provide accurate evaluations. Maintain clear and effective communication with patients and other professionals. Ensure compliance with HCPC / NMC standards while documenting assessments and recommendations. Utilise varied IT systems for record-keeping and reporting purposes, ensuring data accuracy and confidentiality.
