An exciting opportunity has arisen for us to recruit a motivated, experienced and forward-thinking Band 7 Diabetes Specialist Nurse to join our Cross-Bay Diabetes Specialist Care team at University Hospital of Morecambe Bay NHS Trust.
The post plays a key role in the management, care and support of patients with diabetes both within an inpatient and community environment. Visiting and supporting patients and staff in all inpatients areas within FGH .
Applicants will need to demonstrate at least 2 years recent and significant experience as a diabetes specialist nurse to be a driving force within the team.
This exciting role will be to work in partnership with people with diabetes, their families/carers and other professionals, providing a specialist, evidence based and high-quality diabetes care. The post will involve, leading, organising and delivering ongoing education for patients and training for professionals, carers and contribute in the development of local diabetes care strategies and guidelines.
The Diabetes Team covers the whole of Morecambe Bay Footprint therefore flexibility will be required across 3 main hospital sites.
The successful post holder will be required to work as part of the integrated team and will get the opportunity to further develop their diabetes skills across all areas. Fundamentally the nurse will be required to;
· Assess people with diabetes
· Agree and review the diabetes treatment plan for inpatients and deliver nurse led clinics
· Work closely with multi-disciplinary diabetes teams in the hospital and community setting
· Provide support and advise to healthcare professionals on quality evidence diabetes care as per guidelines
· Provide structured education groups providing knowledge and skills to people with diabetes to support their self-management
· Deliver formal and informal diabetes education to health professionals
· Support the development of an integrated diabetes nurse service across primary and secondary care to promote a seamless diabetes care pathway for people with diabetes
· Undertake evaluation and clinical audit to improve patient care and clinical outcomes
· Support an integrated nursing team and work across primary and secondary care settings to provider as required for patient care
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
