East and North Herts Diabetes Service is a nurse-led service. Following the retirement of the current postholder, we are looking for a highly motivated expert Diabetes Specialist Clinical Lead Nurse with a strong desire to improve outcomes for people living with diabetes.
The successful candidate will have extensive experience in diabetes specialist nursing and have the skills and experience to provide strong professional leadership, inspire a team of specialist nurses and dietitians, and provide expertise to the wider clinical staff within East and North Hertfordshire.
Ideally, you will be an independent nurse prescriber with a strong understanding of the legal and professional issues surrounding this role. You will be a champion of patient structured education, and working knowledge of the DAFNE programme will be an advantage.
The postholder will need excellent communication and organisational skills and be able to facilitate clinical staff development both within the team and for appropriate staff in the wider Trust.
You will also be a key stakeholder for the diabetes provision across the Herts and West Essex Integrated Care System, working in conjunction with representatives from the local acute Trusts, primary care, and other providers of diabetes care to shape the strategic direction of provision.
Responsibilities:
* To provide strong professional leadership, advice, support, and direction to the E&N Diabetes Service in the planning, coordination, and evaluation of the service, liaising with the Diabetes Dietetic Lead, Operational Lead, and Assistant Operational Lead where appropriate.
* To be professionally accountable for the Diabetic Specialist Nurses (DSN’s) within the E&N Diabetes Service including clinical supervision, training needs, and professional accountability.
* To deliver a minimum of one clinical session per week (pro rata) within E&N Hertfordshire.
* To actively contribute to the ICB Diabetes work stream ensuring that appropriate links are established and maintained with the Herts Valleys Integrated Diabetes Service (HIDS) and ENHT Diabetes Service to ensure all national initiatives are delivered locally.
* To provide strategic diabetes clinical leadership for HCT within East and North Hertfordshire.

Additional Responsibilities:
* Be the subject matter expert for the E&N Diabetes team, providing expert advice, guidance, and support to the ICT leads.
* To initiate new and maintain existing excellent working relationships with a complex range of contacts, utilising extensive knowledge and skills to influence others in establishing, planning, and implementation within the Diabetes service.
* To steer a process of development and improvement of the Diabetes service in line with national standards and evidence-based practice.
* To champion change and provide a strategic clinical lead for the service.
* Using specialist clinical assessment, clinical reasoning, and highly specialist clinical knowledge, guide and support the DSN’s to formulate and deliver individual treatment plans and self-management programmes.
