Job summary A fantastic and rare opportunity has arisen for a part time community Assistant Practitioner to join the Coventry and Rugby Community Learning Disability Team. This is a split post between nursing and occupational therapy. We welcome applicants with a passion for working with adults with a learning disability and a "can do" attitude, who reflect the values (compassion, respect, integrity, collaboration, excellence) of CWPT. This role provides care and support to those in their homes and services within these catchment areas. We adopt a holistic approach and focus on preventing hospital admissions and placement breakdowns, emphasis on improving client's quality of life. Qualifications Candidates require a Foundation degree in Health and Social Care (Assistant Practitioner) qualification or NVQ (Care) level 3 Four GCSE's grade c or above (Maths and English desirable) or functional skills maths and English (level 2). Experience Experience of working in a Learning Disability practice setting Due to the nature of community working it is essential to hold a full driving licence. Experience of working to support people with a learning disability in distressed states including either significant mental health needs, behaviours of concern or offending behaviour. Experience of working in a multi-disciplinary team in the community. Experience of working with Nurses / Occupational Therapists. Experience of lone workingwithin the community. Main duties of the job Working as part of a multidisciplinary team, assistant practitioners use enhanced skills in a specialised area to provide direct high quality and effective therapeutic care and support to clients including those with more complex needs. Within the scope of the role, the post holder will deliver individual or team tasks where the understanding of role boundaries and the importance of supervision and carrying out appropriately delegated tasks and plans is essential. You will carry a varied caseload of clients and will work in and be supported by the experienced multi-disciplinary.
