An exciting secondment opportunity has arisen for an experienced Band 6 mental health nurse to join the Care Home Liaison Service. The service operates Monday to Friday 8.30am until 16.30pm. The post holder will work within a team of mental health practitioners based in Oldham offering mental health assessment, treatment and behavioural/occupational care approaches, training and advice and support within the Oldham Care Homes and intermediate care facilities. The post holder will liaise with both primary and secondary care services, the wider health and social care footprint, carers and families within their liaison role.

Person specification
* RMN
* Evidence of continuing professional development
Experience
* Experience of working within Older Age Services
* Good working knowledge of physical health problems in the older person and assessment skills in this area
* Knowledge of medication used in this service user group
* Ability to monitor effects and side effects
* Experience of community working
* Skills training in physical health assessment
* Experience of delivering training
Knowledge
* Thorough understanding of dementia and the impact this has on care providers as well as the individual
* Awareness of relevant mental health legislation ie MHA/MCA/DoLS
* Experience of MDT working
* Proven experience in delivering non-pharmacological interventions in dementia with the aim of reducing medication use.
* Evidence of management skills/previous supervisory experience
* Awareness of End of Life issues in dementia care
