Job Overview
An exciting opportunity has arisen for an enthusiastic, dynamic, and motivated General Practitioner to join our Frailty Team working as an integral part of our multidisciplinary team. We are looking to increase permanent senior medical clinicians within our medicine and Frailty teams, keen to include GPwSI in our teams and improve the interfaces between primary and secondary care.
Our team currently comprises ACPs, Nurses, Therapists, Pharmacists, and a GP with the aim of reducing LOS for frail patients, identifying and onboarding onto the virtual ward with Consultant support, and turning suitable patients around from UTC/SDEC, working collaboratively with community and primary care services.
The successful candidate will provide senior review for patients suitable for admission to the frailty virtual ward, delivering a high standard of clinical care while supporting new ways of providing quality services.
The ideal candidate will have knowledge of Care of the Elderly/Frailty, as well as an interest in general medicine or the motivation to learn and develop professionally in a supportive environment.
Main Duties of the Job
The posts are substantive, and essential criteria include Full GMC Registration, entry on the specialist register as a GP plus a special interest in care of the elderly, with motivation to undertake further development and studies in this area.
The role will ensure that frail older people in a ‘crisis situation’ receive expert, responsive, and safe care in a place closest to home, best meeting their needs.
Practice as an autonomous General Practitioner with special interest in Frailty, demonstrating advanced clinical competence and utilizing highly specialized knowledge and skills to diagnose, plan, deliver, and advise on care programs for patients.
Provide clinical leadership, diagnosis, management, and treatment for patients with specialist frailty and end-of-life care needs; act as an expert resource for medical, nursing, and other healthcare professionals across the Trust footprint; and champion high-quality frailty and older persons care both locally and regionally.
The post holder will:
* Accurately assess a patient including history taking, examination, diagnosis, investigation, treatment, and timely referral to multidisciplinary colleagues.
* Provide professional leadership, expert knowledge of frailty management, education, clinical advice, and experience to multidisciplinary team members.
* Maintain accurate clinical records.
