Nurses play a key role in supporting clinical functions within the Confederation sites in the London Borough of Hillingdon.
Main duties of the job
This role involves working in various clinics across our sites in the London Borough of Hillingdon to provide practice nursing appointments, or appointments in specific clinics for which you have the appropriate skills or training, with the potential for some home visits. There is the capacity to broaden the role to have additional input into audits, infection control, planning for clinics, training and support.

Job responsibilities
Key Responsibilities
* In accordance with the services needs the post-holder will make him or herself available to undertake a variety of duties including cervical smear tests, post-operative and basic dressings, HRT reviews, Pill Checks, Immunisations (Baby & Travel if trained), BP checks, Asthma and COPD checks, setting up intravenous drugs, diabetic reviews, Spirometry and Warfarin Monitoring if trained.
* Assessing the health needs of patients with chronic health needs and using your own judgment to assess whether further input is needed.
* Screening patients for disease risk factors and early signs of illness.
* Contributing to coding requirements and recording data appropriately.
* Contributing to PCN requirements and recording data appropriately.
* In consultation with patients and in line with current practice disease management protocols, developing care plans for health-related illnesses.
* Screening patients for health education.
* Admitting and discharging patients to and from the caseload and referring to other care providers as appropriate.
* Recording clear and contemporaneous consultation notes to agreed standards.
* In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a practice nurse working within primary care, as set by Royal College Nursing and the Nursing and Midwifery Council and councils applicable to the staff within the practice, Department of Health, Clinical Commissioning Groups and all other regulatory and standard setting organisations.
Additional Responsibilities
The responsibilities listed below are not essential for the role but are desirable although dependent on the qualifications you may or may not have.
* Collecting data for audit purposes and contributing to the practice audit programme as required.
* Take responsibility for some aspects of the clinical work within the team, for example: coordinating aspects of governance, QOF, enhanced services, CPD and keeping clinical protocols up to date.
* Working in clinics to provide enhanced services for patients in the community.
* Attending multi-disciplinary meetings to interpret results and plan for patient care.

Experience

Qualifications
* Eligible to work in the UK.
* Full registration with the Nursing and Midwifery Council.
* Certificates for Practice Nurse Training and cervical Smears Training with relevant registration numbers.
* Experience working within the NHS or Primary Care.
* Experience or training to work in Enhanced Services Clinics with any of the following specialisms:
* Travel Vaccinations and Advice.
* Spirometry and Reversibility.
* FENO.
* Cannulation and administering IV medications.
