Main duties of the job The role is an all-rounded,clinical ACP role; the postholder will need to confidently evidence and utilisekey skills such as ability to work on own initiative, ability to develop anevidence-based, personalised, integrated health and social care plan inpartnership with the individual, carers, relatives and health careprofessionals, based on a full assessment of medical, nursing and social careneeds, and demonstrate ability to act as an autonomous advanced practitionerproviding a comprehensive clinical assessment service topatients/carers/families resulting in safe, appropriate clinical decisions.
