We are looking for an enthusiastic and driven individual to become a member of the Site Management Team as Patient Flow Assistant on a permanent basis. You will assist and provide requested support within all areas of patient flow, enabling the hospital sites to provide efficient and effective patient flow, helping to reduce barriers to patient flow and supporting the Trust in providing a gold standard service enhancing our patients' experience.
The Patient Flow Assistant will be expected to work as part of the Site Management Team under the direction of the Patient Flow Manager, to ensure the smooth flow of patients through the wards and departments across the hospital. By co-ordinating administrative tasks, and liaising with other departments and the site team, the post holder will contribute to efficient and effective patient flow, resulting in a significant reduction in patient journey times.
The post holder will be required to assist the Patient Flow Managers within the Site Management Team to plan and facilitate a timely discharge process for patients from point of admission to the ward environment to the discharge of patients to their own home or alternative care settings.
You will have excellent communication and interpersonal skills to ensure close working relationships exist within the acute setting at various management and clinical levels and with key partners across the Health and Social Care networks.
Main Responsibilities:
1. Assist the Patient Flow Managers to plan and facilitate a timely discharge process for patients.
2. Establish and maintain effective working relationships with staff at all levels, both internal and external to the organisation.
3. Support in highlighting patients admitted to the ward with current packages of care and feedback to the integrated discharge hubs, ward staff, and individual agencies.
4. Act as the first point of contact in the absence of the Operations Centre Administrator, prioritising communications into the services.
5. Receive, quality check and process Discharge to Assess referrals for NLaG hospitals out of the Operations Centre Administrator working hours.
6. Work closely with the wide MDT including relevant partner agencies to facilitate discharge and contribute to discharge planning alongside the discharge liaison team.
