Role Purpose: You will work flexibly across the borough of Dudley, working and engaging within a range of communities and partners. Delivering Health checks to general public between the age of 40 and 74 particularly those from the most marginalised groups, within community and clinical settings. You will lead on training and competency checks for existing staff and partner agencies to deliver health checks, this may include working with partners outside of the Dudley borough. The role will require knowledge of policy implementation, development of SOPs and Auditing against robust frameworks. You will be a motivated, passionate, organised and proactive communicator, with experience of working in a community setting. Duties and Responsibilities This post will work under the line management of the Head of Service. The role will be varied, challenging and rewarding as you support and develop the service through implementation, delivery and ongoing innovation and service development. Key responsibilities will be: Ensure the NHS Health Checks service is of high profile and there is strong awareness in relation to the criteria of entry in the service. Assist the Head of Service to develop the integrated health offer within Dudley and raise the profile of ABL Health. Identify and engage with individuals from target groups and communities to facilitate communication regarding promoting their health and wellbeing, and the Lifestyle service. Attend health events across the borough, undertake health checks and promote the wider Dudley service Develop and maintain relationships with individuals who are experiencing the greatest inequalities in health and provide information to individuals about the relationship between behaviours and health. Make contacts, develop and maintain professional links within communities including promoting the Health Checks Service and your role to contact groups where there may be barriers to understanding. Liaise with key partner organisations such as, workplaces, community organisations, community centres and faith groups to increase referrals into Your Health Dudley services. To support all the Your Health Dudley services to ensure quality and governance is robust Promote the integrated Lifestyle Service and all components within the local community To champion and adhere to processes and procedures to identify and strive for continuing improved quality throughout the patients pathway, together with robust data management. Deliver against an agreed set of KPIs. To meet internal and external performance management requirements Be responsible for ensuring rigorous and regular data inputting to ensure accurate records are maintained The post holder is expected to work within the requirements of the 6 Cs Care, Compassion, Competence, Communication, Courage and Commitment. Train existing and new staff to delivery Health Checks Annual competency and observation checks for all staff delivering Health checks Attending local health forums/groups/meetings where required to represent ABL, promote the service and maintain local partnerships Support the Head of service to develop robust SOPs Support the Head of Service to complete 10% audits of all work carried out Engage with GP surgeries and Primary care networks to develop case finding and population health management projects to drive referrals into all elements of the service Develop partnerships with secondary care and inpatient services to increase smoking cessation referrals Skills and Competencies Required Embed referral pathways to ensure efficiency and quality referrals into the service Training expertise Experience of building and maintaining community partnerships Build strong relationships; Foster trust and co-operation among colleagues, stakeholders, community leaders, commissioners and sub-contractors Quality driven; you naturally seek high standards and actively seek to improve them.
