We have an exciting opportunity fora fixed term,Band 6 Community Learning Disability/Mental Health Nurserole, in the Trust-wide Young People’s Specialist Support Team.
This multiagency team consists of clinical psychology, learning disability nursing, speech therapy, occupational therapy and support workers. The Team sits alongside CCLDT and CAMHS, and works as a step up service beyond local community provision.
The Team work with a small number of young people with severe learning disabilities and/or autistic young people with complex needs including behaviours of concern, whom require intensive support. Working alongside local respite and short breaks services, the Team work to understand the young person, their interests, strengths and needs, to create, instigate and support person-centred positive behaviour support plans to improve quality of life and reduce restrictions and restraint.
This is a chance to help to deliver a service based on the principles of the Ealing Model, including positive behavioural support and trauma informed interventions. You will support the Team with the continued mobilisation of this model across our five localities, linking to the dynamic risk register and working alongside families.

The postholder will form part of a multidisciplinary team consisting of clinical psychology, learning disability nursing, speech and language therapy, occupational therapy and support workers.
The post holder is responsible for providing nursing assessment, diagnosis and behavioural treatment for children with learning disabilities and complex needs (including autism, behaviours of concern, and other long term conditions) in a variety of settings including special schools, the child or young person’s home and community clinics.

• To ensure high quality functional assessment of young people and families referred to the YPSST
• To provide access to a range of quality interventions in accordance with individual assessed need and in line with a person-centred, positive behavioural support approach. This will include assessment, planning, implementation and evaluation of a variety of approaches as part of the MDT.
• Assessments may be face to face or virtual and must show appropriate consideration of medical, cognitive, psychological, social, environmental, and cultural factors
• To be able to undertake clinical interviews with children, young people and their families or carers in order to gain a holistic person-centred understanding of the young person’s experience and needs, quality of life, environment and aspirations.
• Manage a caseload, offering a range of clinical assessment and evidence based behavioural interventions to meet the needs of children and young people with learning disabilities and complex needs, under supervision.
• To undertake analyses of behaviour, interpreting and integrating data from a variety of sources, including observations, self report measures and questionnaires.
• To formulate cases as part of the MDT based on an appropriate assessment to inform the Positive Behaviour Support plan.
• To co-create person centred Positive Behaviour Support plans with the team around the child or young person, taking the lead in some plans and having a supporting role in others as needed. The plans need to explicitly draw out how the environment may need to change to support the young person and how their quality of life is going to be improved through implementation of the plan. The plans need to incorporate proactive as well as reactive strategies and explicitly address restraint and restriction.
• To take responsibility for communicating with the referrer, young person and family and other agencies involved as required.
• To undertake liaison and joint work with other professionals outside of the team
• To provide training, advice, and consultation to staff from health and other agencies in areas relevant to direct clinical work and service provision
• To routinely monitor the effectiveness of clinical work through use of Routine Outcome Measures (ROMs) and Patient Stories, reformulating and update plans as necessary and ensuring this is communicated to all parties involved.
• To recognise barriers to change and act on these as appropriate.
• To engage in managerial and clinical supervision and deliver clinical supervision (as appropriate) to more junior nursing staff.
• Ensuring collection of relevant information required by Pennine Care Foundation Trust
• To deliver difficult, highly sensitive or contentious information to families, carers and professionals, making adjustments where there are barriers to communication and understanding
• To independently assess risk and create risk management plans as required.
• To be responsible for following step up and step down procedures around PBS plans with the Specialist Community Service for Learning Disabilities and Autism as required.
• This role will require the postholder to work across quadrants 3 and 4 of Thrive including risk management.
