Job Summary
The Stroke Service on The Heartlands site can offer an exciting development opportunity for someone who already has high level knowledge and skills and now wishes to specialise in Acute Stroke.
We are recruiting an ACP to join the ward and non-ward-based stroke team. Stroke values the ACP workforce who have been working in our team for many years.
The ACP will deliver a comprehensive service for potential stroke patients, who present via The Emergency Department, telephone referral or internal referral.
The ACP is the first line of contact for internal and external teams and is responsible for triaging need and prioritising management including onward referrals.
The successful candidate will be a qualified ACP who is looking to specialise in stroke. They will work in several environments including the acute ward environment, high risk TIA clinic, follow up clinics, new assessment in ED and inpatient ward environments.
The stroke service at Heartlands is developing and this is the beginning of a workforce growth to meet the new challenges in the service as we rebuild following significant changes during and after COVID. Main Duties of the Job
* The post holder will be a competent ACP.
* The post holder will deliver 90% clinical component to their role.
* The post holder will work in close conjunction with the Stroke Clinical Service Lead, the Stroke Consultant Nurse, the advanced practice team and the ward to provide a seamless pathway for patients from the point of admission through to discharge and in clinics afterwards.
* The postholder will be the initial contact for most external and internal referrals and requests for information.
* The rest of the role is within the JD and PS attached.

Person Specification
Qualifications Essential
* Professional Healthcare Registration with a UK Regulatory Body (NMC / HCPC / GPC)
* Post Graduate Qualification in specific speciality or equivalent experience.
* MSc in Advanced Clinical Practice/ Level 7 equivalent portfolio or minimum of 120 CATs and working towards full MSc (must cover all 4 Pillars of advanced clinical practice) -- Portfolio must include evidence of health assessment module at Level 7
* Non-Medical Prescribing Course
* Advanced Life Support Qualification
* Recognised Teaching & Assessing/Mentoring Qualification/Course
Experience Essential
* Extensive post registration experience at a senior level in specialty area including significant and relevant senior clinical and leadership experience.
* Evidence/experience of working in an MDT within a healthcare provider service.
* Evidence of ongoing CPD.
* Evidence of teaching in a variety of environments.
* A sound knowledge of professional priorities and issues relating to NHS care provision.
* A knowledgeable clinician with ability to ensure clinical governance is embedded in practice.
* Sound understanding of evidence-based practice and its application.
