We are looking for an imaginative and enthusiastic person to join our team supporting 5 Males in Harkstead. You will be working within their home, so its important you can connect, share interests and be interested. We are looking for people who enjoy getting out into the wider community but are equally ready to get stuck into activities back at home too.
Main duties of the job
The people we support enjoy car rides, exploring the local car boots, the cinema and the odd game of football. If you share these interests, we want to hear from you!
You should be available to work a variety of shifts across the week and some weekends. We currently have available Full Time, Part Time and Bank Positions.
An essential part of this role is being a driver and having access to your own vehicle due to its rural location.
So, if you enjoy building meaningful relationships and think you could plan, communicate and deliver supportwithandalong sidepeople, then this is the place for you.
Affinity Trust does not presently hold a Sponsorship Licence and while this status is reviewed regularly there is no present proposal to apply for such a Licence
What will you do?
Supporting people is really important but its not all swimming and crafts. We also support people to take medication, assist with personal care, make healthy lifestyle choices and maintain their home in a way they are safe, well and happy.
It sounds exciting to say no two days will be the same, but in support, sometimes they are. Sometimes people like routine and struggle with change But when you support someone to try something new what a feeling!

Job Purpose
To support people with learning disabilities with all aspects of their daily lives, ensuring thatthey have maximum opportunities and choice for developing new skills and leading fulfilling,varied, are valued and live socially inclusive lifestyles where their rights, dignity and safetyis protected.
Key Responsibilities
1. To identify and develop with each person an outcome based support plan, riskassessment and person centered plan which are reviewed and updated regularly
2. To assist the people we support with routine personal and domestic tasks, includingpersonal care, household tasks and cleaning and promote a healthy lifestyle whenshopping, meal planning, cooking and choosing activities
3. To monitor and promote each person's health and well being, ensuring that eachperson has a Health Action Plan, that medical help is sought when needed, dental,eye, hearing and other check-ups or treatment are undertaken and that changes inhealth and wellbeing are reported
4. To assist the people we support to participate in their chosen social, educational,leisure and sporting activities in the local community, taking into account theirculture, faith, gender and sexuality and where desired, to support individuals toaccess volunteering or employment opportunities
5. To assist the person to improve his/her knowledge, skills and abilities throughplanned learning activities and experiences
6. To enable the people we support to keep and make new friends and keep in contactwith family, friends and people in the neighbourhood and develop a circle of support
7. To enable the people we support to participate in the running of the service, offeringmeaningful choices and supporting them to make decisions which affect them andother
8. To administer medication when required, in accordance with policies and procedures and after completion of medication assessment
9. To liaise with other people involved with the person we support, including GP, CareManager, family, day opportunities and other agencies

Qualifications

1. UK Driving Licence and business insurance (if specified for the role)
2. Willingness to work towards Qualifications & Credit Framework Diploma (QCF) or Level 2 Scottish Vocational Qualifications (SQV) and complete all sector training.
