Job responsibilities Clinical responsibilities: Assess and triage patients, including same-day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team. Manage undifferentiated undiagnosed conditions and identify redflags and underlying serious pathology and take appropriate action. Use complex decision-making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice. Actively take a personalised care approach and population-centred care approach to enable shared decision-making with the presenting person; and Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, such as first contact practitioners and personalised care roles. Provide the choice of direct access to a nurse practitioner, both in the Practice and over the telephone, for the general practice population. Make home visits to those who are too unwell to come into the clinic in person. Make professionally autonomous decisions for which you are accountable. Provide assistance to GPs seeing and treating patients referred to you as appropriate. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports. Prescribe safe, effective and appropriate medication as defined by the current legislative framework in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate. Provide safe, evidence-based, cost-effective, individualised patient care. Offer a holistic service to patients and their families, developing where appropriate an ongoing plan of care/support, with an emphasis on prevention and self-care. Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills. Refer patients directly to other services/agencies as appropriate. Work with the team within the Practice, and across the wider NHS, to promote integrated and seamless pathways of care. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery. Participate in the identification of community health needs and develop patient/family-centred strategies to address them. Help develop and set up new patient services and participate in initiatives to improve existing patient services. Educations & Professional Qualifications - Essential: Masters qualification for Advanced Clinical Practice or similar accreditation. Independent Prescribing qualification. NMC registration. Experience & Skills Essential: Ability to prioritise and organise own case load. Excellent communication and interpersonal skills. Ability to evaluate and implement appropriate clinical service developments. Ability to use in house Patient Databases. Experience of working within a team. Willingness to undertake further training as required. Evidence of Continued Professional. Desirable; Experience working within a PCN. Specific Aptitude and Abilities Essential: Self-motivated.
