The Gateshead Learning Disability Team are happy to be offering an exciting opportunity for Band 5 Community Nurse.
We are a multidisciplinary community team working across the whole of Gateshead with individuals with complex needs. The successful post holder will be expected to demonstrate a strong commitment to high quality patient care, good organisational skills and be able to demonstrate prioritisation of workload. Key components of the role will be to ensure the service offers a safe, caring, responsive and well-led service.
The role will include liaising with agencies such as social care, service providers, inpatient services, safeguarding, alongside a range of other agencies. Within this team we pride ourselves on having a broad range of knowledge within these selected areas and the ability to manage the risks associated with this.

Main Duties of the Job
1. To collaboratively engage in the assessment and management of a caseload of patients who have substantial and complex needs.
2. Provide clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.
3. Provide high quality assessment and care, alongside offering the wider team support, advice and supervision as required.
4. Work collaboratively with patients to develop care plans, recovery and wellbeing plans, ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary.
5. Consider carer needs and ensure carer and family involvement in care planning where appropriate.
6. Deliver evidence-based clinical interventions.
7. Work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery, wellbeing, and maximising independence.
8. Coordinate treatment planning, interventions, clinical reviews, and discharge planning.
9. Provide clinical supervision to staff as delegated by Pathway Manager and Clinical Leads.
10. Provide compassionate care that is based on empathy, kindness, respect, and dignity.
11. Incorporate a patient-centred philosophy into nursing practice and patient-centred collaborative care.

Person Specification
Qualifications
* Experience of working with service users and carers in successful delivery of treatment plans.
* Complete preceptorship programme for newly qualified nurses.
* Multi-disciplinary team work.
* Proven ability to develop therapeutic partnership with service users and carers.
* Proven ability to communicate interventions to service users.
* Further training relevant to the post.
Knowledge and Experience
* Knowledge of relevant contemporary professional theories of mental health care and evidence-based interventions.
* Understanding of the statutory issues which impact on service users.
Skills and Competencies
* Ability to use IT systems.
* Comprehensive continuous assessment of skills.
* Risk Management and Safety Planning skills.
* Ability to establish rapport and demonstrate engagement skills with service users.
* Adaptability, flexibility, good communication skills, and a commitment to multi-agency teamwork.
