1. Work within primary care, multi-disciplinary team settings and VCSE Organisations to support services to become more digitally enabled.2. Deliver virtual and face to face training to individuals and groups regarding the use of digital technology and its application within improving and maintaining health and wellbeing.3. Produce a range of accessible resources to support individuals and communities in the use of digital technology in addressing health and wellbeing needs4. Engage communities to consider health inequalities and use digital means to create bespoke support addressing the wider determinants of health. 5.Be responsible for own time management, working autonomously and as part of a team to facilitate training, 1:1 support and information guidance across services, communities and organisations.6. Developing and maintaining relationships with key stakeholders including primary care, VCSE organisations, and community representatives to support effective service delivery.7. Engage in side by side working in order to co-produce a service that is responsive to individual and community needs.8. Support the delivery of key performance indicators for the service9. Producing reports as requested.10. Keep accurate records, update systems and databases, and maintain digital platforms to a high standard in order to produce robust and timely statistical reports.11. Signpost people to other services where appropriate and support access to these to gain further support around health and wellbeing needs.12. To actively participate in developing directorate, team and individual objectives and plans, attend staff performance management and learning and development reviews.13. The post holder is expected to be flexible in the performance of duties and to undertake any other duties identified as appropriate to the post.14.
