Job summary An exciting opportunity has arisen for three 36hr positions within the Blaenau Gwent Community Resource Team (CRT) as reablement Support Workers. Blaenau Gwent CRT is a multi-disciplinary service provided by Aneurin Bevan Health Board and Blaenau Gwent Social Services. The CRT provides support to adults, over the age of 18 living in Blaenau Gwent who want to stay independent, within their own homes. The team comprises of an Intermediate Care Consultant, Speciality Doctor, Rapid Response nurses, Occupational Therapists, Physiotherapists, Speech & Language Therapist, assistant practioner's and reablement support workers. Main duties of the job You will work as part of a team providing a person centred approach to support the person with a rehabilitation programme in their own home and will support the assessment and provision of therapeutic interventions and activities for each individual. Previous experience of working with people in a care delivery capacity is desirable. All posts are subject to an enhanced Criminal Record Bureau check.
