As a Health Liaison Recovery Facilitator, you will support Bracknell Forest residents facing physical or mental health challenges from drug or alcohol use. By working with hospitals and GPs, you will create tailored support plans to improve health and wellbeing. This role offers personal growth and the satisfaction of helping others. About the team… New Hope is the hub for Drug and Alcohol services, and this role is crucial in ensuring timely assessments for those seeking support. As part of a dedicated team, you willl help provide Bracknell Forest residents with the highest quality care. The successful applicant will join a motivated, collaborative team committed to delivering the best support possible. Key responsibilities Build strong links with hospitals and GPs, visiting in-patients. Refer individuals to medical services, supporting them in attending appointments. Facilitate group programs based on service needs. Take a lead role in networking with agencies, developing specialist knowledge and interventions. Manage a caseload, coordinating care and supporting access to BBV, Sexual Health Services, and medical appointments. Extensive training provided to enhance knowledge on substance misuse.
