Floating Support Coordinator
Location: Manchester
Salary: A£24,020 - A£30,790 per annum
The Organisation: The organisation launched in October 2024 following the merger of two other organisations. Their vision is to break down the barriers that stop people getting the support they need to live a life they value. They tackle poverty and disadvantage in communities, through mental health, drug and alcohol, housing and related support.
The Role: As a Floating Support Coordinator, you will have the opportunity to support clients through one-to-one work, care planning, risk assessment, and multi-agency working. You will also be involved in delivering group interventions and liaising with their partners to support each client's overall health and well-being. This role is based in Manchester and involves visiting clients in their homes to deliver support. Therefore, a valid UK driving license is required.
Key Duties:
1. Collaborate with colleagues, community organisations, and stakeholders across a range of settings to help individuals maximise their sustainable recovery capital and community opportunities.
2. Deliver an evidence-based, comprehensive psychosocial program alongside a range of activities that support wellbeing.
3. Ensure effective delivery of a support package within the full integrated service model, focusing on successful and sustainable exit from support.
