We have a fantastic opportunity to join the CALDT team as an Advanced Practitioner.
The Advanced Practitioner role provides day to day operational management and support for team members within the CALDT team. The team works with Adults with Learning Disabilities and/or Autism, aged 18 and over, primarily under the Care Act and the Mental Capacity Act.
The Advanced Practitioner will promote best practice standards based on an up-to-date knowledge of social work practice developments and relevant policy and legislation. You will hold an appropriate qualification, Dip OT, BSc OT, DipSW, CQSW, CSS or equivalent Management qualification and have experience of strength-based working across agencies to deliver Social Care interventions and service developments. You will have experience of working with adults with Learning Disabilities and/or Autism.
You will provide 1-1 supervisory support to an identified group of staff members within the team and offer support on processes such as legal interventions and mosaic, and will work closely with other teams within the council and partner agencies to promote an ethos of strengths based, person centred, community led support.
As an Advanced Practitioner, you will provide professional casework advice, and expert social work knowledge along with support and guidance to colleagues including newly qualified Social Workers, trainees and other team members and/or volunteers on cases including the most complex cases within the parameters of agreed policies and practices.
You will oversee and advise on caseloads providing holistic, strengths based assessments which are person-centred to help adults, carers and families make informed choices and decisions, enabling them to clarify and express their needs to achieve outcomes, manage risk and contribute to service planning.
The post-holder will be an experienced and motivated individual who is able to support practitioners in their role and to develop their skills. The post-holder will be expected to partake in a 7 days on call service on a rota basis providing management cover for the Integrated Discharge Team.
The post holder will report to the Team Leader of the Community Adult Autism and Learning Disability Team.
