Due to a service review, an exciting opportunity has arisen to join our Twilight Out of Hours Urgent Care Service band 6 coordinator.
The post is fluid in its location, we would ask you to go where the need is greatest on a shift. That may be Chichester, Arundel or Lancing. You will be visiting patients in their own homes between the hours of 18:00-23:00hrs but starting at 17:00hrs as part of a rota being the shift lead, supporting other twilight teams across West Sussex.
The short twilight shifts are ideal for part-time workers. We work a four-week repeating rota so that you can plan your life well ahead of time. In addition to being a very family-friendly shift pattern, one of the many benefits of working the evening shift is that all hours are paid with the appropriate enhancements.
Community experience is essential as you will be leading on complex out of hours care. The successful candidate will be professional, enthusiastic, adaptable & will have to be a positive role model.
Every shift on the Out of Hours is different: You will lead on End of Life Care, manage complex catheters, deteriorating patients whilst supporting staff on shift. You will work alongside other B6 nurses ensuring the immediate and wider team are supported.
Main duties of the job
1. Providing urgent nursing care to patients referred into the service.
2. When coordinating, supporting all Twilight teams on shift across West Sussex (mainly phone support).
3. Working flexibly to move to different locations when the area has lower staffing levels (travel included in shift time).
4. Liaising with the Clinical Nurse lead and the Operational team Lead to ensure smooth running of the service.

Job responsibilities
Together with your Team, you will deliver high quality nursing care in the community setting out of hours, providing whatever short term input is needed within the scope of practice. In liaison with our Day Team colleagues, we aim to support & maintain our patients in their own homes.
The post holder as part of the Team will provide the care needed whatever that may be. It might be enabling end of life care for the patient to ensure they achieve their preferred place of death, supporting them, their family & friends to meet that goal. Alternatively, with the Team, you'll be enabling those living with urinary catheters to live independent lives or dealing with whatever urgent health/nursing need may crop up.
The Band 6 shift co-ordinator will manage the clinical needs of the referred patients for that shift. The Service provides all & any urgent nursing care needs which cannot wait until the next day using skilled assessment of need & providing interventions before arranging with Day Teams & other care services for the on-going care of patients as required.
You will need to be a confident driver as travelling at night and adverse weather conditions are common. We provide a pool car for use but you will also need to have your own car available for some shifts.
