Responsibilities
Provide direct support to clients, assisting them with daily living activities.
Develop and implement individualized care plans tailored to client needs.
Facilitate community integration and engagement opportunities for clients.
Monitor and evaluate client progress and provide regular feedback.
Communicate effectively with families, social workers, and other stakeholders.
Assist clients in accessing community resources and services.
Maintain accurate and confidential records of client interactions.
Requirements
Relevant qualifications in social care or community support.
Previous experience in a similar role or working with vulnerable populations.
Strong interpersonal and communication skills.
Proficient in creating and implementing care plans.
Ability to work independently and as part of a team.
Knowledge of local community resources and support services.
Flexibility to work varied hours, including evenings and weekends.
Hours Per Week: 32.50
Start Time: 09:00
End Time: 15:30
Pay Per Hour: £14.84
Location: Enfield, London
