Our heart failure specialist nurse team play a pivotal and dynamic role in the care of our patients with heart failure. We have developed excellent systems of care, for example automated NTproBNP referrals and a heart failure clinical pathway, and are integrated with an excellent multi-disciplinary team including the community and palliative care teams. You will be working in a team caring for acute inpatients primarily, from first diagnosis to end stage heart failure. Your role will be focused on ensuring medicines are optimised, providing lifestyle and self-care advice to patients and carers and ensuring the patient has a comprehensive management plan on discharge. You will be preparing patient information for medicine and device MDTs and inputting data into the national heart failure database (NICOR), You will receive a comprehensive induction programme, will be supported by band 7 HFSN and follow the BSH competency framework to guide development in your role.
