Advanced Nurse Practitioner - Hambleton South
Heartbeat Primary Care CIC have recently formed a General Practice Urgent Care Hub and are looking for an ANP to support facilitate the contract. The successful ANP will work across three local practices in the Hambleton South PCN area. The hours will be 12 per week spread across 3 days, days to be agreed. This is a fixed term contract for 1 year.
Main duties of the job
To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced ANP and in addition any roles agreed between Heartbeat Primary Care CIC and local doctors as appropriate, having regard to current training.
Job responsibilities
You will play a vital role in assessing, diagnosing, planning, and implementing care for the new Urgent Care Hub contract.
* Conduct both face-to-face consultations in clinics, and telephone consultations.
* See patients of all ages
* Treat minor illness and/or Injury trained
* Prescribe and review medication, ensuring adherence to treatment plans.
* Play a key role in service development and quality improvement initiatives to enhance care delivery.
Person Specification
Experience
* Significant experience in working as an ANP within primary care
* Ability to work autonomously
Other
* Full valid driving licence and have access to travel to various locations in the local area.
Qualifications
* Hold MSc in Advanced Clinical Practice
* NMC Registered
* Degree at 2:1 level or above
* Possess independent prescribing qualification
