The role
will be to review existing packages of care, ensuring that they are meeting individual’s needs and outcomes and maximising people’s independence.
We work alongside a range of professionals within the teams, including colleagues from the health board, therapies and children’s services It is an exciting time to join adult social care with friendly, experienced and supportive colleagues who are dedicated to providing a service that puts citizens at centre of our work.

Ideally you will have;
Applicants will need a reasonable standard of education to GCSE (or equivalent) level. Along with a related experience of working with people with social care needs in the communities they live in, with families and with their social networks, either in a paid or voluntary capacity.
A qualification related to the Social Work or Caring Professions e.g. BTec Social Care, Nursing, NVQ and some knowledge and understanding of principles of the Social Services and Wellbeing (Wales) Act 2014 would also be an advantage.
