Job summary
Joining our Reablement Team as a Community Reablement Assistant Level2, youll help to support adultsover the age of 18 across Hampshire who may be experiencing a need foradditional care and support. Our aim isto maximise their independence and potential, as well as reduce, avoid or delaythe need for ongoing care whilst supporting them to remain living at home.
Our team of Community ReablementAssistants, Occupational Therapists and Sensory Officers, work inpartnership with Health colleagues to offer support, encouragement and guidanceto those with short or long term conditions andfrailties as well as supporting those coming out of hospital.
Main duties of the job
What youll do:
1. Drive to service users' homes and working independently carry out reablement intervention.
2. Work towards enabling service users independence in daily living tasks and personal care
3. Deliver hands on care
4. Write up medication
5. Undertake risk assessments
6. Complete and review service users goals
7. Complete working supervisions with Community Reablement Assistants Level 1s and support their development
8. Use a variety of IT systems such as MS Teams and email.

9. Experience of working within a social or health care setting
10. Effective communication skills both verbal and written
11. Sound literacy and numerical skills
12. Sound IT skills
13. Motivated and work on own initiative
14. Good observation, listening and feedback skills with the ability to seek advice from other appropriate professionals
15. Good team player
16. Able to prioritise delegated workload and demonstrate effective time management skills
17. Leadership skills
18. Willingness to be inducted and trained in enabling skills

About us
Working Patterns
Our Service runs 07:00-22:00, 7 days a week, 365 days of the year, therefore youll be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours for which enhancements are paid in line with the HCC salary policy. Details of the working patterns available will be discussed in more detail during the selection process.
