Specialist Mental Health Practitioner (OPMH RMN/RGN)

We are looking to recruit a highly skilled, enthusiastic individual to join us as a key member of our multi-disciplinary team working closely with Urgent Community Response to ensure that the physical and mental health needs of older adults are addressed. You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards.
Providing urgent response to older adults who are experiencing a health crisis to support the most unwell individuals living in care homes, and this post provides a new and exciting opportunity to use your knowledge and expertise to reduce avoidable hospital admissions, minimise placement breakdown and reduce the use of anti-psychotic treatments. There is also the opportunity to shape service provision and personally develop.

Qualifications
Essential
1. Appropriate relevant professional Mental Health Nurse, SW, or OT
2. Evidence of other development and/or training which indicates a commitment to continuous development and preparation for management
3. willingness to undertake Nurse-prescriber qualification or pharmacology training
Desirable
4. Masters or Additional post-graduate qualification in dementia or related to mental health care.
5. Quality improvement, management, research or leadership qualification and/or training
6. Nurse-prescriber qualification or pharmacology training.
7. Psychological therapy qualification or training.
Experience
Essential
8. In depth experience in the effective and positive management of complex and high risk cases
9. Ability to demonstrate a commitment to multi-disciplinary team working
10. Must demonstrate significant post-qualification practitioner experience
11. Demonstrate the ability to work collaboratively with other agencies
12. Able to illustrate examples of service development initiated by the candidate
13. Willingness to demonstrate the potential for 'change management' approaches which extend existing practices
14. Exposure to, understanding of and competence in IT facilities and systems associated with service users, budgets and management information
15. Evidence of experience as a supervisor, including a clear understanding and application of an effective caseload management system
16. Demonstrate planning skills, in terms of resource allocation and funding
17. Demonstrate an understanding of, and a commitment to the principles of recovery, choice and personalisation
18. Commitment to attend staff training and participate in own development through supervision and performance development.
Desirable
19. Previous Experience of Liaison Psychiatry work, in a service which spans the adult and or older age group.
20. Experience of working in inpatient or community OPMH teams at Band 6 level.
21. Experience in acute care (crisis/AMHT) at Band 6 level.
