Job Overview
An exciting opportunity has arisen to be part of the New Community Enablement and Recovery Team (CERT) for the Black Country. This role includes providing wrap-around care in conjunction with CMHTs and care coordination of service users from a capped team caseload that are either in out-of-area rehabilitation placements or living within the Black Country. These individuals will be open to secondary care services with a diagnosed Severe Mental Illness and requiring community rehabilitation support.
The post holder will also be responsible for supporting discharge from hospital, providing recovery-focused interventions and assessing the complex mental health needs of individuals, ensuring that the placement and/or package of care is appropriate to their needs and step down to locality based services. The post holder will work intensely with the client for up to 18 months, focusing on helping to build resilience, improve their wellbeing and tailoring interventions to meet their individual, cultural and social needs to help them to achieve their optimum level of functioning and a fulfilling life.
Main Duties of the Job
1. Provide care coordination, wrap-around input to CMHT, assessment and recovery-focused interventions to service users within a specified time frame across the Black Country.
2. Liaison with external services to facilitate discharge from out-of-area placements, coordinate 117 packages of care and support referral process.
3. Contributing to the development and implementation of the Community Enablement and Recovery Team (HOPE team).
4. Working within an MDT to work holistically and improve our patient's recovery journey.
Person Specification
Experience
1. Community experience.
2. Experience of working with individuals with severe mental illness.
3. Knowledge of person-centred, recovery-focused and trauma-informed care.
4. Experience of working within a Recovery Model.
Knowledge
1. Multi-professional teamwork.
2. Mental Health Legislation and policy including CPA and 117.
3. Commitment / Genuine interest of client group within area of speciality.
4. Ability to prioritise workload.

The salary range quoted is as per NHS Agenda for Change pay structures. This is based on the number of years working directly within the NHS at the advertised pay band. Experience gained with other NHS Trusts will apply, but candidates without prior experience working directly for the NHS would normally be appointed at the lowest point unless they can demonstrate the equivalent experience and then be eligible for incremental increases in future years.
Successful applicants for clinical posts up to and including Band 4 are expected to hold the national Care Certificate qualification and will be supported to attain this once in post.
