About the role We have an exciting opportunity for a motivated Care Navigator to join our team on a full-time, permanent basis at PRUH UTC. Please note that this role will involve evening/ weekend working and is based on a rota model supporting the UTC opening hours. Your Day to Day… The role of Care Navigator, based within our Urgent Treatment Centre, is to be patient focused, supporting UCC staff to communicate the appropriate use of urgent care pathways within the centre. You will receive patient information following assessment from a GP or ECP/ENP, supporting patients to ensure they feel informed, supported and listened to so that they can make meaningful decisions and choices about their care. You will support the UTC staff in collecting information and feedback from patients about their experience, and share this information with the CCG to improve community outreach and to achieve the key performance indicators. Qualification, Experience & Training… Educated to NVQ Level 2 or equivalent Working knowledge of Microsoft Office including Word, Excel and Outlook.
